What it is

Spirinolactone is a brand of spironolactone, which belongs to a class of medicines called aldosterone antagonists. It works by blocking a hormone that can cause fluid retention and high blood pressure.

What it's used for

Spirinolactone is used for several conditions. It is prescribed for adults with NYHA Class III–IV heart failure and reduced ejection fraction to improve survival, manage fluid buildup, and reduce the need for hospital stays. It is also used as an add-on treatment for high blood pressure (hypertension) to lower blood pressure and reduce the risk of strokes and heart attacks. Additionally, it is used to control edema (swelling) in adults with liver cirrhosis when other measures haven't worked, and for primary hyperaldosteronism (a condition where the adrenal glands produce too much aldosterone).

How it's taken

Spirinolactone is taken by mouth as a tablet. The dose your doctor prescribes will depend on the condition being treated. For heart failure, treatment typically starts with a low dose taken once daily. For hypertension, the dose may range from once to twice daily. When used for edema, doctors usually start treatment in a hospital and adjust the dose slowly. For primary hyperaldosteronosis, doses are often higher and used before surgery or as long-term therapy. Always follow your doctor's instructions and do not change your dose without medical advice.

Common side effects

Possible side effects include high potassium levels (hyperkalemia), low blood pressure (hypotension), worsening kidney function, electrolyte and metabolic abnormalities, and breast enlargement in men (gynecomastia). Your doctor will monitor you for these effects with regular blood tests.

Important cautions

Spirinolactone should not be used if you have high potassium levels, Addison's disease (adrenal gland failure), or if you are taking eplerenone (another similar medicine). Before starting treatment, tell your doctor about any kidney or liver problems. Your doctor will check your potassium levels within one week of starting the medicine and regularly thereafter. Also monitor blood pressure, kidney function, and electrolyte levels periodically. In patients with liver cirrhosis and ascites, spironolactone can cause impaired neurological function or coma, so careful monitoring is required.
