What it is

Alpadactone contains spironolactone, which belongs to a class of medicines called aldosterone antagonists. It is a type of diuretic that helps the body remove extra fluid and also lowers blood pressure by blocking a hormone called aldosterone.

What it's used for

Alpadactone (spironolactone) is used for several conditions: to treat heart failure (NYHA class III‑IV with reduced ejection fraction) – it helps patients live longer, controls swelling (edema), and reduces the need for hospital stays; as an add‑on therapy for high blood pressure to lower the risk of stroke and heart attack; and to manage edema in adults with liver cirrhosis when other treatments have not worked.

How it's taken

Alpadactone tablets are taken by mouth. The dose depends on the condition being treated and is determined by a doctor. For heart failure, the starting dose is typically 25 mg once daily. For high blood pressure, the usual starting dose ranges from 25 mg to 100 mg daily, taken as a single dose or divided into two doses. When used for edema, treatment often starts in a hospital with 100 mg daily. For primary hyperaldosteronism, the dose before surgery is 100–400 mg daily. Always follow your doctor’s or pharmacist’s instructions.

Common side effects

Common side effects of spironolactone include high potassium levels in the blood (hyperkalemia), breast enlargement in men (gynecomastia), low blood pressure, and worsening kidney function. Other possible effects are electrolyte imbalances, increased uric acid, and changes in blood sugar. If you have liver problems, it may affect your brain function.

Important cautions

Do not take Alpadactone if you have high potassium levels, Addison’s disease, or if you are already taking eplerenone. Your doctor will check your potassium level within one week of starting treatment and regularly afterward. They will also monitor your blood pressure, kidney function, and electrolytes. Tell your doctor about any other medicines you take, especially those that can raise potassium (like certain blood pressure drugs or potassium supplements).
